I hereby authorize the repair work hereinafter set forth to be done along with the material and agree that Statkus Engines are not responsible for loss or damage to vehicle or articles left in vehicle in case of fire, theft or any other cause beyond your control or for any delays caused by unavailability of parts or delays in parts shipments by the supplier or transporter. Statkus Engines is not responsible for coach damage that may occur from lifting or removing access covers or plastic that may crack due to age. I hereby grant you and/or your employees’ permission to operate the vehicle to operate the vehicle wherein described on streets, highways, or elsewhere for the purpose of testing and/or inspection. An expressed mechanic's lien is hereby acknowledged on above vehicle to secure the amount of repairs thereto. Units left over 72hrs will be subject to $10 per day storage fee. I hereby acknowledge that I am fully responsible for all freeze damage resulting from unit being left wet in winter. I acknowledge that the minimum charge to diagnose is $180.00. An estimate for repairs will be given upon diagnosis. I agree to pay for any and all charges authorized.
- Labor rate per man hour is $180.00
- Customer must have all storage compartments & windows locked.
- Door must be able to lock and key provided to lock entry door.
